Child Death Benefit

This benefit is paid out on the death of a covered child. If a claim is approved, the amount will be paid out to the policyholder as a lump sum.

Which documents do I need for to file my claim?

A completed claim form.
A clear, certified copy of the deceased’s ID.
A copy Notice of death/DHA-1663.
A clear, certified copy of the official death certificate issued by the Department of Home Affairs.
A certified report by deceased’s regular medical practitioner.
A clear, certified copy of your ID.
Policyholder’s bank statement, stamped by the bank (Note: the statement cannot be older than three months at the date the claim is submitted).
We may request further medical information and/or documents, if required.
